- Click “Cremation Authorization” button. Form will open in new browser window, SignNow.
- Read the form, complete fields, and submit. Close SignNow browser window.
- Return to browser tab for www.CapitalCityCremation.ca/online-secured-form/
- Complete Acknowledgement Form fields
- Click “Submit” button
